Transforming Healthcare with Digital Solutions

Conative Health designs and builds digital ecosystems to deliver hybrid and virtual health services. We leverage our experience to co-design services, transform models of care, and build patient pathways to solve the health issues of today and tomorrow.
Our solutions reduce costs, drive revenue, increase capacity, and improve patient health outcomes.
Our customers are  public and private health providers, private health insurers, primary health networks  and clinical practices.

Why we stand out

Cost Reduction

Our custom built digital ecosystems help reduce costs by optimizing workflows, automating tasks, and streamlining processes. Duplication of effort is removed,  tasks are automated, errors are reduced.

Integration with exisiting mangement software further reduces administrative burden and decreases time spent on low value but necessary paperwork. Overhead costs are minimised while capacity is increased.

Access & Outcomes

With our digital and hybrid models of care, patients benefit from increased accessibility, personalised care plans, remote monitoring, and secure messaging, leading to an enhanced patient experience.

Patients receive necessary clinical support without the inconvenience of synchronous episodic appointments contained by time, receive earlier discharges and supported recovery at home, contributing enhanced patient experience.

Increase Capacity

By leveraging automation  in conjunction with   wearables, telehealth, customised survey assessments and digital triage tools, our solutions deliver continuous remote care and enable self-assisted patient care.

This technology enabled automation allows  valuable clinical resources to work at top-of-scope, increasing capacity, and reducing workload. Enabling providers to deliver high-quality care to more patients.

enhanced HITH Capacity

Expand capacity and reduce overhead costs

Enhancing HiTH models with automation technology  allows the clinical team to remotely monitor vital signs, and provides continuous and real-time assessment against documented trends and established parameters.  In combination with automated protocols supporting patient self management, the clinical care team work at top of scope, focusing on complex decision-making and interventions.
Our digitally enhanced HiTH model reduces in person home visits without compromising care, increasing the admission capacity of HiTH teams to care for more patients without increased workload or resources requirements.

Waitlist Audit and Management

Automated audit and dynamic monitoring

Automated administrative audit of entire waitlists ensures accuracy and transparency for hospital administrators removing labour intensive manual audit processes. Clinical audit and monitoring from the point of referral allows for dynamic prioritisation of waitlists based on clinical need.  Shared communication with Primary Care referrers enables shared monitoring and real time understanding of deteriorating patient health status or changes in health requirements.

ED Diversion

Est 10-20% presentation reduction

Substantial numbers of known patients with exacerbation of existing conditions present to ED departments every day.  Using remote monitoring and automation these patients can be instructed with self care guidance and have early intervention initiated by primary them presenting to ED. The estimated reduction in ED presentations is from 10-20% dependent on use case.

Outpatient Clinic Capacity

Est 25% capacity and funded activity increase at marginal cost

The average specialist consulting new to review ratio of 1:4 can be reduced by 25% or more, by converting episodic synchronous reviews to continuous review using remote monitoring and automation.  Activity based funding is available for these remote care models to maintain funding levels and clinic capacity is increased as review appointments are vacated enabling new patient booking and waitlist reduction.

Bed Block

Reduce LOS and prevent readmission

Transition of care (TOC)  is one of the most challenging and siloed areas of health.  Remote monitoring of vitals, signs and symptoms combined with automation, and communication tools, and real time information sharing with Primary Care, extends clinical service outside the traditional hospital setting to provide continuous seamless care, patient engagement and optimised resource utilisation.

Virtual Rehabilitation

100% increase in participation and completion rate

Rehabilitation programs  improve health outcomes, however participation and completion rates remain low for in centre programs.  Our virtual solutions have proven to double and triple both participation and completion rates preventing readmission and improving outcomes at a fraction of the cost of in centre care. Virtual cardiac, pulmonary and orthopaedic programmes have delivered significant revenue and capacity increases to health providers while improving patient experience.

GP Access And capacity

Replacing episodic synchronous consultations with continuous automated care, GPs can be paid for their knowledge not just their time while patients receive ongoing continuity of care.  The availability of real time and longitudinal information shortens consultation time while enhancing the value and quality of every interaction allowing improved revenue for the GP and increased access to appointments for more patients.

care at home

The use of remote patient management provides home care providers with increased capacity and reduced cost of care delivery while improving patient care.  A  large range of integrated devices including personal monitored alarms provide peace of mind and ongoing support for families and home care providers.

RACF support

Real time sharing of vitals, biometrics signs and symptoms with offsite medical staff and the use of integrated remote diagnostic devices and tools prevents unnecessary transfers to hospital and improves patient care.  Medication prescribing, supply and management become seamless and connected.
